We receive referrals from:
- Other health-care providers (e.g., neurologists, neurosurgeons, primary care physicians)
- Mental health providers (e.g., psychiatrists, psychologists, social workers, licensed counselors)
- Licensing and regulatory organizations (e.g., FAA, professional medical associations)
- Attorneys, fiduciaries, Guardians/Conservators, Powers-of-Attorney
- Family members and care-givers
We also accept self-referred clients. We value and appreciate mutually respectful and collaborative relationships with our referral sources.
Assessment and diagnostic services provided by AZN include:
- Baseline screening of neurocognitive functioning
- Assessment of memory disorders
- Diagnosis-focused evaluation for assessment of dementia and related conditions
- Comprehensive neuropsychological evaluation following acquired injury or stroke
- Geriatric neuropsychological evaluations
- Evaluations to determine Competency, Guardianship, and Conservatorship needs
- Medical-legal / forensic neuropsychological and psychological evaluations
(Civil, Criminal, Fitness For Duty, and IMEs)
- FAA neuropsychological evaluations (including CogScreen if needed)
Treatment services provided by AZN include:
- Neuropsychologically-oriented psychotherapy and psychoeducational therapy for clients coping with challenging neurological disorders
- Psychoeducational therapy for family members coping with a loved one's neurological disorder
- Adjustment to cognitive and emotional problems accompanying medical and neurological illness
- Advocacy and referral services for ancillary assessment, treatment and follow-up care
